Payment Agreement

Before we begin services, please sign below indicating you have read, understand and agree to the following payment policies.

You agree to be financially responsible for all charges regardless of any applicable insurance or benefit payments, third-party interest, or the resolution of any legal action or lawsuits in which you may be involved.

WBD does not participate with any insurance providers. However, clients may use their HSA/FSA depending on client policy's terms and conditions.

Payment is expected at time of service unless you have made other payment arrangements with us.

Cancelation Policy

Wellness Services

We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all our clients, the following policies will be honored:

There will NOT be a fee applied to a missed or canceled appointment IF you or your child is sick, or there is an act of god (i.e. weather, accident, injury, etc.). Practitioners will not show up under these circumstances either and therefore we do not expect you to. If you are ill, please do not come in for treatment and call us to reschedule your appointment. There are times where appointments need to be rescheduled on the practitioners’ behalf due to the above listed circumstances as well. Although this does not occur often, we understand that this can be an inconvenience and apologize in advance.

Cancellations within 48 hours of appointment time will be charged/invoiced 50% of the service fee.

When canceling or rescheduling an appointment within 24 hours the client will be charged/invoiced 100% of their service fee.

We value our time as much as our clients and want to make sure we honor both.

Service Packages

These packages are to be used within 12 months of purchase and are non-refundable.

Late Arrivals

Clients are encouraged to arrive 5-10 minutes prior to their scheduled appointment time.

In the event that a client arrives late (after the scheduled start time) the service will conclude at the original time in order to respect the next client. However, accommodations will be made when at all possible and at the therapists’ discretion

Gift Certificates

CAN be purchased online anytime or in store with an appointment

CANNOT be split between clients (IE: buying 90 minutes and splitting time 45 minutes each)

EXPIRE 5 years after purchase

DECREASE in value 10% every year after purchaseCAN be regifted (but still expire and lose value based on time of purchase)

CANNOT be redeemed for cash or returned/refunded.

Client understands that the massage service offered is for the therapeutic purpose of general wellness, stress reduction, and relief of muscular tension.

Client understands that this is a therapeutic treatment and NON-SEXUAL in nature and if at any point the therapist feels uncomfortable, the session will be terminated immediately, the client will be invoiced and the authorities will be notified.

Information about massage therapy, potential benefits, effects, risks, contraindications, and possible alternative therapies have been explained to me and the client understands this information.

Client understands the risks associated with massage therapy include, but are not limited to:

  • Superficial bruising

  • Short-term muscle soreness

  • Exacerbation of undiscovered injury

If Client experiences any pain or discomfort, they will immediately inform the therapist so that the pressure or techniques can be adjusted to an appropriate comfort level. They will not hold my massage therapist responsible for any pain or discomfort they experience during or after the session.

Client has provided my therapist with an accurate and complete medical history and agrees to inform the therapist of any new diagnoses, or changes in health or medications. Client does not have any injuries or conditions that prevent them from receiving massage therapy.

Client understands the importance of informing my massage therapist of all medical conditions and medications that they are taking, and that there may be additional risks based on my physical condition.

Client understands that they or the massage therapist may terminate the session at any time.

Client releases the massage therapist and business from all liability for any harm that may unintentionally result from this treatment.
